Stevenson MarchersThe Stevenson High School Marching Band is proud to have built a long tradition of excellence. The band performs annually at MSBOA Festival and Utica Community Schools' Band-a-Rama and has participated at regional marching activites throughout Metropolitan Detroit. While away from Michigan, the SHS Marching Band has performed in Orlando & Miami Florida and Washington, DC. The band represents Stevenson High School in Sterling Heights, Michigan, USA.

Registration & Payments for the 2020-21 Stevenson Marching Band

 

Sending your child away to camp is a big event! To assist our outstanding group of volunteers help provide the safest environment and provide the best care possible for your student while they are away from home, we ask that you invest the time needed to complete the following registration steps thoroughly and accurately.

Stevenson away band camp is licensed through the State of Michigan; in addition to these registration steps being the right thing to do for the kids, it's required by law. We cannot leave for away camp until 100% of this paperwork has been been completed (and to the satisfaction of our camp Health Care Officer).

With approximately 200 people attending our week of away camp each year, your consideration in doing your part goes a long way in not only helping with preparations but in the care of your child, especially if the unexpected happens.

Step 1: Register online at: www.stevensonbands.org/2020MarchingBand

  • Registration opens Wed. March 25, 2020 at 7PM EDT
  • Registration due on or before Mon. May 4, 2020 to ensure:
    • Space at Away Camp (housing and busing)
    • A spot on the field (if not in Frontline)
    • A 2020 show theme shirt included in price
  • Complete Step 1 AND pay a $100 non-refundable deposit (Step 2 below) no later than Fri. May 1, 2020 to receive $25 off the $450 Marching Band fee for 2020
  • Completion of both Step 1 and Step 2 by Frday, May 1st qualifies eligibility for a second $25 discount, bringing the Marching Band Fee down to $400, should medical paperwork be completed and turned in (Steps 3 - 5 below) by Wednesday, June 10 and full payment made by Friday, August 7

 

Step 2: Pay a $100 non-refundable deposit online towards the $450 Marching Band Fee at: www.stevensonbands.org/2020MarchingBandPayments

  • Payments accepted beginning Wed. March 25, 2020 at 7PM EDT
  • Deposit due on or before Mon. May 4, 2020 to ensure:
    • Space at Away Camp (housing and busing)
    • A spot on the field (if not in Frontline)
    • A 2020 show theme shirt included in price
  • Complete Step 2 AND register online (Step 1 above) no later than Fri. May 1, 2020 to receive $25 off the $450 Marching Band fee for 2020
  • Completion of both Step 1 and Step 2 by Friday, May 1st qualifies eligibility for a second $25 discount, bringing the Marching Band Fee down to $400, should medical paperwork be completed and turned in (Steps 3 - 5 below) by Wednesday, June 10 and full payment made by Friday, August 7

 

Step 3: Download, print, complete (must be notarized), and turn in the Emergency Medical Treatment form

  • Completed/notarized form due on or before June 10, 2020
  • Emergency Medical Treatment Form - Complete fields online, then save and print from your computer and present to a notary for signature acknowledgment.
  • Form is used in case your child is taken to a medical facility outside of camp for treatment. Presenting a notarized form gives the medical facility parental permission to provide care and greatly improves efficiency / reduces wait time.
  • Completion of Steps 3, 4, & 5 by Wednesday, June 10th maintains eligibility for a second $25 discount, bringing the Marching Band Fee down to $400, should full payment made by Friday, August 7th. Discount eligibility also depends on Steps 1 and 2 (above) being fulfilled by May 1st.
  • Should the information on this form need to be updated after it was submitted, a replacement form may be submitted at any time prior to departure to away camp without the loss of any discount previously earned. Recommended: keep a photocopy / take a picture of this form prior to submission.

 

Step 4: Download, print, complete, and turn in the Medical & Allergy Information form

  • Completed form due on or before June 10, 2020
  • Medical & Allergy Information Form - Complete fields online, then save and print from your computer.
  • This form has TWO (2) sides. Be sure to turn over and complete side two, including student and guardian signatures.
  • Click here for instructions and assistance with this form. This information is especially important to review for guardians of students attending away camp for the first time.
  • Form is used while your student is under the care of the in-camp nursing staff and/or in case your child is taken to a medical facility outside of camp for treatment. Providing accurate, detailed, and legible information greatly assists in the care of your student while they are away from you.
  • Completion of Steps 3, 4, & 5 by Wednesday, June 10th maintains eligibility for a second $25 discount, bringing the Marching Band Fee down to $400, should full payment made by Friday, August 7th. Discount eligibility also depends on Steps 1 and 2 (above) being fulfilled by May 1st.
  • Should the information on this form need to be updated after it was submitted, a replacement form may be submitted at any time prior to departure to away camp without the loss of any discount previously earned. Recommended: keep a photocopy / take a picture of both sides of this form prior to submission.

 

Step 5: Photocopy the front and back of the medical insurance card(s) the student is covered under and turn them in

  • Photocopies due on or before June 10, 2020
  • Military ID cards may be substituted, if appropriate.
  • Copies of insurance cards are only used should your child be taken to a hospital, urgent care, or clinic.
  • The birthdate of the insurance card holder is required by all health care providers in Michigan (provided on the form in Step 4 above). Should your child receive care outside of camp, you may be contacted to obtain the insurance card holder’s Social Security number by the health care facility. The Band will not collect Social Security information.
  • Should you not have medical insurance and/or a medical insurance card:
    • Provide a photocopy of the front and back of your driver’s license
    • Include a typed statement that is addressed to “Health Care Provider” explaining that you are either self-insured or not insured but accept financial responsibility for medical care that is provided to your child. Sign and date this statement and return it in place of the copy of the insurance card.
  • Completion of Steps 3, 4, & 5 by Wednesday, June 10th maintains eligibility for a second $25 discount, bringing the Marching Band Fee down to $400, should full payment made by Friday, August 7th. Discount eligibility also depends on Steps 1 and 2 (above) being fulfilled by May 1st.
  • Should insurance information / cards change after submission, replacement copies may be submitted at any time prior to departure to away camp without the loss of any discount previously earned.

 

Step 6: Complete payment of the Marching Band Fee at: www.stevensonbands.org/2020MarchingBandPayments

  • Final and complete payments due on or before August 10, 2020
  • Completion of
    • Steps 1 + 2 above on or before Friday May 1st &
    • Steps 3 + 4 + 5 above on or before Wednesday, June 10th &
    • Total payments of $400 towards the Marching Band fee (not including apparel costs) on or before Friday August 7th
    activates a $50 total discount (Marching Band fee reduced from $450 to $400)
  • Outstanding balances after August 14, 2020 are subject to being added to the student's Power School account as a fine. Students with fines on their accounts will not be allowed to:
    • Participate in optional student travel opportunities
    • Purchase tickets to optional student activities such as the Homecoming Dance and Prom
    • Receive their cap and gown and participate in their high school graduation ceremony
  • Marching Band is a pay-to-play optional activity that is primarily funded through a combination of fees and parent-directed fundraising. Participation in Marching Band requires payment.

 

Step 7: Download, print, complete (must be dated on or after April 15, 2020 and physical section completed out by a medical professional), and turn in a Physical form

  • Completed form due on or before August 10, 2020
  • Simplified Physical Form for Band Students - Complete fields online, then save and print from your computer and take to a medical professional to complete during a sports physical.
  • UCS rules concerning sports physicals (positively NO exceptions):
    • Must take place on or after April 15, 2020
    • Must be completed by a medical professional
    • Every Marching Band member must have a sports physical completed to participate in both home and away camps
  • Marching Band students that will be (or are thinking about) participating in an UCS athletics activity in the 2020-21 school year MUST substitute an official MHSAA physical form in place of the simplified band member physical form so that copies are given, by you, to both band and athletics
  • UCS athletes are responsible for providing their own copies of their physical form to both band and athletics. The band does not provide copies of physicals to any other organization - this is YOUR sole responsibility!
  • UCS, or any of its schools or organizations, does not maintain a centralized filing system for physicals. Each organization operates independently.
  • Should the information on this form need to be updated after it was submitted, a replacement form may be submitted at any time prior to departure to away camp without loss of eligibility. Strongly recommended: make and keep a photocopy of this form prior to submission, whichever version is used.

Receiving Physical Education Flex Credit for completing 2 full seasons of Marching Band: A student may be able to waive the graduation requirement of 1/2 credit of Physical Education (gym class) following the completion of two seasons of Marching Band between 9th and 12th grades. In addition to 100% participation, the proper paperwork must be completed, signed by Mr. Sekich, and returned to the counseling department to complete eligibility.
